The Aged Care Assessment Team is for Home Care Packages, Transitional Care, Respite and Residential Care.
They provide assessment for older people who need some help at home or are considering moving into aged care.
Assessors talk with the older person about their current situation and work out if they are eligible to receive government subsidised aged care services.
