Peer Led Aftercare Service - Nepean Blue Mountains Local Health District (LHD)
For people leaving hospital after an attempt to end their own life. Assistance and support from a Peer Worker who has a lived experience of suicide is available to help the consumer when transitioning home and into the community to engage with support programs and relevant services for ongoing care.
