Health Connectors are available in seven general practices in the Hawkesbury area to help people 75 and over (or 65 and older for Aboriginal and Torres Strait Islander people), living in their own home or independent living, with making new connections.
Health Connectors are trained nurses who will talk with you one-on-one to see what may suit you and give you information about local services like classes, social or support groups, volunteer programs and transport options to get there.
