caring@home develops and provides nationally consistent, practical and evidence-based clinical resources and education for health professionals. This supports:
caring@home resources are free of charge and are applicable Australia-wide for clinical services, health professionals, and families and carers.
caring@home is funded by the Australian Government.
Patients or families should contact their local palliative care service or GP about making use of these resources.
These resources will support quality end-of-life care during the last 12 months of life.
