The Plains Older Persons Community Mental Health Team provides a range of trauma informed and recovery orientated services designed to help support people aged 65 years and over impacted by a major mental illness to live and stay well in the community. People under 65 years old, including Aboriginal people aged 50 years and older, can also use this service if it is suitable for their needs.
Our Older Persons Community Mental Health Teams achieve this by working collaboratively with the person to identify a range of interventions and activities aimed to improve a person’s wellbeing, their ability to live in their own homes, social and relationship skills and achieve broader quality of life via physical health, social connectedness, housing, education, and employment. This approach enables the team to provide the appropriate level of care to best support the person during the various stages of recovery.
