This is a funded occupational therapy (OT) service for older adults living at home in Lithgow. The service includes an initial in-home assessment, where needs will be identified and an intervention plan developed. Follow-up support including assistance with My Aged Care applications, making referrals for home modifications and equipment is also provided as part of the service.
Eligible people must be aged 65 years and over (or aged 55 years and over for Aboriginal and Torres Strait Islander peoples) and they must reside in the community or in independent living units/over 55s living in the Lithgow Local Government Area (LGA).
Priority is given to people who are either in the CHSP program, awaiting a higher level package or have not yet engaged with My Aged Care. If you/your client is the recipient of an active Home Care Package/ Stay at Home Package, please make contact using the form below prior to submitting a referral.

This service is fully funded by Wentworth Healthcare, the provider of the Nepean Blue Mountains Primary Health Network. This service is delivered by Live Well Lithgow.
